I don't usually see any other doctor other than my family doc, but lately I've had to see a few medical specialists. They send a health questionnaire in advance.
I was surprised to see these questions:
Are you sexually active? Yes No Not currently
Sex of Your Partner (check all that apply): Male Female Both
Birth Control Methods Used: About 15 were listed
When did this start? I don't remember these kinds of upfront questions in the past. I don't like this because lots of people look at your medical records.
I'd lie on the form (and then tell the doctor him/herself in person if I had a sexual-related issue). I've filled out dozens of these forms (though none with questions this explicit). In my experience, the doctor doesn't even look at the form, since the doctor always seems to ask the same questions him/herself (related to whatever complaint you're presenting).
